Patient Agreement

Welcome to Skinscape Clinic. We are committed to providing quality medical aesthetic treatments and we want to partner with you to achieve the best outcomes for your skin and health. Please carefully read and acknowledge the following Terms and Conditions which apply to all our patients.   1. General Terms and Conditions

1.1 These terms and conditions form the basis of the agreement between the patient (you/your) and Skinscape Clinic (us/our).

1.2 By registering with us, you agree to be bound by these terms.

 

  1. Consultation

2.1 As a new patient, you can only proceed with treatment after receiving an initial consultation with our doctor. If, after consultation, no treatment is to be offered or if you do not wish to proceed with treatment, only the consultation fee will be payable.

2.2 Our practitioners are not obliged to treat you if they deem treatment is not appropriate for any reason.

 

  1. Consent for Medical Care

3.1 Before receiving certain treatments or procedures, you may be asked to complete a consent form. If you refuse to sign such a consent form, that treatment or procedure will not be performed.

3.2 You voluntarily consent to medical treatments, procedures, tests, medication and all other services performed or prescribed by us. You understand that you have the right to refuse consent to any proposed treatment, procedure, test or medication.

3.3 Once treatment has commenced with your consent, you shall be liable for full payment of the treatment.

 

  1. Medical Photography

4.1 You agree that photographs of you may be routinely taken to document your appearance and skin condition, and to monitor the progress and results of your treatments.

4.2 Your photographs will form part of your confidential medical records and will be securely stored.

4.3 You may request for a copy of your photographs at any time.

4.4 Occasionally, you may be asked to give consent for the photographs to be used for patient education during consultation in Skinscape Clinic. Your photographs will be redacted so that it is unlikely you will be identified. You are free to refuse.

 

  1. Prices

5.1 Prices for our treatments, procedures, services, products and medication are subject to change without notice at any time.

5.2 We reserve the right at any time to modify or discontinue any treatment, procedure, service, product and medication without notice at any time.

 

  1. Disclaimer and Limitation of Liability

6.1 You acknowledge that the treatments, services and products offered by Skinscape Clinic aim to improve your skin, appearance and/or health concerns, and the outcomes of such treatments, services and products are subjective. You acknowledge that we do not guarantee the results, as results may vary between individuals. Factors such as lifestyle, medical history and age can affect your results and the longevity of such results.

6.2 It is your responsibility to ensure that you provide our practitioners with all relevant medical history and information about pre-existing medical conditions, including pregnancy. We shall not be liable for any loss or damage, including personal injury, sustained as a result of your failure to disclose such details.

6.3 By proceeding with a treatment or procedure, you agree to comply with all instructions and advice given to you by our practitioners or on behalf of Skinscape Clinic regarding the care of a treated area.

6.4 Nothing in herein shall exclude or limit our liability for death or any personal injury resulting from our negligence.

 

  1. Refunds

7.1 While we will do our best to ensure a satisfactory outcome, we do not offer a refund for any completed treatment or procedure unless we acknowledge that the treatment or procedure rendered was below the professional standard we strive to achieve.

7.2 Treatment or procedure fees will not be refunded in the case of a sub-optimal result (including no result) or complications, as all fees paid are related entirely to the services provided i.e. professional time, expertise, product costs and other costs incurred in providing the treatment or procedure, and those costs are incurred irrespective of the results achieved. Additional costs may be incurred should complications arise from the treatment or procedure and further treatment is required.

7.3 Medicine or medical products sold are strictly non-refundable and non-exchangeable.

7.4 All other products can be returned for an exchange to an identical item or refund if the product is established by us to be defective and returned to the Clinic within five (5) working days of purchase.

 

  1. Etiquette and Restrictions

8.1 If you are under the age of twenty-one (21), you should be accompanied by your parent or guardian for treatments. If your parent or guardian is unable to come, you should obtain their written approval before proceeding with treatment.

8.2 You assume the risk of loss or damage of your personal items, including money, brought into Skinscape Clinic. We shall not be liable for any loss or damage of any such items.

8.3 We reserve the right to refuse treatment or service to any patients who behave in an ill-mannered, aggressive or hostile way towards any member of our staff.

 

  1. Feedback and Complaints

9.1 We welcome all feedback, both positive and negative. Feedback is used to review and improve the quality of our service and treatments. You may submit your feedback during the normal course of communication with our staff or via this link https://tinyurl.com/yckd396a.

9.2 While we always use all reasonable endeavours to ensure that your experience as a patient is a positive one, we nevertheless want to hear from you if you have any cause for complaint. We strive to resolve any complaint you may have regarding the care, treatment or service provided by us in a fair, confidential and timely manner.

9.3 If you have a complaint about the care, treatment or service you have received at Skinscape Clinic, you may contact us by telephone or WhatsApp. We will listen carefully to your concerns and do what we can to correct any problems and provide a suitable solution.

9.4 Alternatively, you may write in to our Patient Care Services for resolution at contact@skinscapeclinic.com. We will review your complaint and provide a response within five (5) working days.

 

  1. Communication:

10.1 By providing an e-mail address and phone number in our Patient Registration Form, you authorise Skinscape Clinic to communicate with you via e-mail and/or text message at the provided e-mail address and phone number. Such communication may include your personal data and sensitive information (e.g. Payment Invoice).

10.2 You acknowledge that e-mails and text messages are not necessarily secure mediums for sending or receiving personal data and sensitive information, as they are susceptible to interference or interception by third parties beyond our reasonable control. Accordingly, you acknowledge that we cannot guarantee the absolute confidentiality of these communications with you.

10.3 You acknowledge that it is your responsibility to take appropriate steps to keep your personal data and sensitive information sent by us to you and your electronic devices secure.

10.4 We shall not be liable for any loss or damage (including economic loss) or for any other special, indirect or consequential loss or damage arising from any breach to the security, integrity or confidentiality of any communication with you via e-mail and/or text message, save if such loss or damage is caused by our failure to maintain adequate security measures.

10.5 At our discretion, emails and/or text messages with you may be made a part of your confidential medical record.

10.6 You acknowledge that e-mail and text messaging are not appropriate means of communication in an emergency or for time-sensitive problems. In an emergency, or a situation that you could reasonably expect to develop into an emergency, you understand and agree to go to the nearest A&E, and follow the directions of emergency personnel.

 

  1. Collection, Use and Disclosure of Personal Data

11.1 You agree and consent that we may use, process and disclose your personal data (that you had previously provided, that you now provide and that you may in the future provide) for the purposes of (i) providing medical care to you; (ii) dealing in any matters relating to our services, treatments, products and activities; and (iii) facilitating, administering, managing and/or maintaining your relationship with us.

11.2 We are committed to protecting your privacy and personal data. Our Privacy Policy, available here, provides further details on our personal data protection policies and procedures, in line with Singapore’s Personal Data Protection Act. You are deemed to have read and understood our Privacy Policy.
